EMERGENCY DATA FORMS
An emergency data form must be completed for each student who attends St. Anthony School. This includes important telephone numbers, after-school caretakers, individuals who may pick-up your child/children, and procedures to be taken during an emergency. Parents must keep this information updated for the safety of their children.

MEDICATION GUIDELINES
According to State and Archdiocesan guidelines, the school cannot distribute medication without the authorization of your physician. This includes any over-the-counter medications. Information from our Health Aide, Miss Tammy, has been enclosed in your packet.

STUDENT DIRECTORY
Each year a directory is assembled containing the student's name, grade, parents' names, address, and telephone number. This year we will, also, include your e-mail address. These directories are distributed to students, faculty, and staff. If you do not want this information or any part of it included, please inform the school in writing.

The address of the student's primary residence will be included in the directory. If other addresses should be included, please inform the school.

NATIONAL SCHOOL LUNCH PROGRAM
Free and reduced lunch assistance is available for those who qualify. If you have questions about this program, please contact Sister Joann. All information completed on the form is highly confidential and all identities are strictly protected.

LUNCH
Lunch is available everyday to all the students. School lunch is $2.00 per day. Money should be sent the first day of each week for meals that will be eaten that week. Meals are to be paid for in advance so that meal accounts are kept current and accurate. When a student brings lunch, milk can be purchased for 35¢.

BIRTHDAY TREATS
Striving to adhere to, and continuing to implement our wellness policy, we are requesting that birthday treats contain less sugar content. We are encouraging parents to bring more nutritious treats for the children. Suggestions are: fruits, fruit snacks, ice cream, sherbert, milk, fruit popsicles, popcorn, veggies and dip, granola bars, etc. These are only a few ideas to help you in creating some nutritious snacks for the children.

CHILD ABUSE FORMS
Anyone wishing to attend a field trip, volunteer at school, or assist with any type of school activity must complete three steps:

1. Have a Child Abuse Form on file with the parish. This form is required to protect the students at St. Anthony School. A $9.00 fee and a copy of your social security card must accompany the form.
2. Attend a "Protecting God's Children Workshop". One of these workshops will be offered at St. Anthony School on August 12, 2008, from 6:30-9:00 PM. Additional workshops are available; please see the enclosed brochure. Click for Workshop Schedule
3. Read, sign, and return the commitment from of the Archdiocese of St. Louis Code of Ethical Conduct for Clergy, Employees, and Volunteers Working with Minors. These form are available in the office.

For more information, call the rectory (573-468-6101) and ask for Marcia.

UNIFORMS
Uniforms are worn in all Grades K-8. The uniforms worn at St. Anthony can be purchased through the local suppliers: Dickey Bub Shop and Sports Image. A black, brown, or dark blue belt is considered part and parcel of the uniform. Students who do not wear a belt will be considered "out-of-uniform".

"Students are expected to dress and present an appearance consistent with standards of good taste and appropriate for school and school events. Any dress or wearing of insignia which conveys the image of gang membership, support the beliefs of hate groups, makes sexual innuendoes, or promotes drugs, alcohol, or tobacco is inappropriate for school and will not be allowed".
